If you are interested in our license plate program, please read through these steps and then click on the license plate picture at the top of the page. It will move you over onto our secure server for purchasing.
STEP 1: Make an annual contribution of at least $25.00 to Choose Life of Missouri, Inc. through our online system via Visa, Mastercard, Discover, American Express or Debit.
STEP 2: We will generate an Emblem Use Authorization Form for your vehicle. Choose Life of Missouri will send to you this authorization along with the appropriate Department of Revenue Application Form (DOR Form 1716). If you need a "Physicians Statement for Disabled License Plate/Placard" (DOR Form 1776), you will have an opportunity to indicate this need, and we will send you the necessary form.
STEP 3: Complete your application and send it along with your Emblem Use Authorization statement and the annual DOR $15 plate fee to the Missouri Department of Revenue. A pre-addressed envelope will be provided to you. The $15.00 is a State required fee for all special and personalized license plates. Please make this check payable to "DOR".
STEP 4: DOR will notify you by mail when your plates are available and provide information about acquiring them at your local license contract office. You will receive a notification letter when your license plates are ready for pickup. The Department of Revenue asks that you please allow up to six weeks after the license plates are ordered for the notification letter to arrive.
Important - For Your Information:
-You may obtain CHOOSE LIFE plates at any time regardless of when you license plates expire. All Missouri specialty and organizational license plates are have a fixed expiration month of July. No matter what month you have, DOR will change your month to July.
-If you would like to see if a license plate lettering configuration is available for your license plate, please email your idea to mvbmail@dor.mo.gov, and someone from DOR will be able to assist you.
-If you are someone who already has personalized license plates, your letters can be put onto the Choose Life license plates, however you will have to surrender your current license plates when you pick up you CHOOSE LIFE license plates.
-Choose Life license plates fit up to 6 letters, or 5 if you have disabled plates.
-Whether applying for or renewing Choose Life license plates, a $15 personalized plate fee is assessed. If applying for two years, the first $15.00 is due when the DOR Form 1716 is submitted as stated in the previous sentence, and the second $15.00 is due to the license office when your plates are picked up at the license office.
-When picking up your Choose Life license plates from the license office, nominal one-time fees will be charged by DOR in conjunction with your license plate. The fees range from $3.50-$4.25 each. Some of these fees vary depending on your license plate type and the specific circumstances of your license plate. For more information, please contact DOR at (573) 751-4509.
